LIABILITY WAIVER     MODEL RELEASE     CANCELLATION POLICY     RESCHEDULING POLICY     PRIVACY POLICY

LIMITATION OF LIABILITY:      I hereby waive, disclaim and release Cute Moose Photography, LLC and its respective owners, members, employees, agents, volunteers and representatives from any and all actions, claims or demands that I may have now or in the future, whether known or unknown, against the Cute Moose Photography Parties, including without limitation those related to injury to or death of persons, or damage, loss or destruction of property arising out of or in connection with the photography session or any products related thereto.

INDEMNIFICATION:      I hereby agree to indemnify and hold harmless the Cute Moose Photography, LLC Parties from all liabilities, losses, claims, costs (including attorney’s fees and court costs), damages or expenses of whatever nature that may result from injury to or death of persons, or damage, loss or destruction of property arising out of or in connection with the photography session or any products related thereto. 

PARENT / GUARDIAN WAIVER FOR MINORS (Under 18 years old):     The undersigned parent and natural guardian does hereby represent that he/she is, in fact, acting in such capacity, has consented to his/her child or children participating in the photography session, and has agreed individually and on behalf of the child or children, to the terms of the accident waiver and release of liability set forth above. The undersigned parent or guardian further agrees to save and hold harmless and indemnify each and all of the parties referred to above from all liability, loss, cost, claim, or damage whatsoever which may be imposed upon said parties because of any defect in or lack of such capacity to so act and release said parties on behalf of the minor and the parents or legal guardian.

MODEL RELEASE:     I hereby give permission to Courtney Cassat (Cute Moose Photography, LLC) to use my name, my child’s name and our photographic likenesses in all forms and media for advertising, displays, trade, and any other lawful purposes.

Cancellations will be refunded at 50% of the purchase price, minus any discounts or promotions.  There is no charge if you need to reschedule your photo session.  

In the event of a miscarriage or infant mortality, Newborn Session purchase price will be refunded 100%.

There is no charge to reschedule.

PRIVACY POLICY:    Cute Moose Photography, LLC is the sole owner of the information collected on this site. We only have access to information that is voluntarily shared via form submissions, email or other direct contact from you. We will not share, sell or rent this information to anyone.   Infrequently, we may contact you via email in the future to tell you about our services, promotions or changes to this privacy policy.  You may opt out of this communication at any time.  For credit card transactions, we will never see, collect or store your credit card information.  Our web hosting provider and credit card processing merchant provides our site with a 128 bit SSL encrypted checkout which is Level 1 PCI compliant.  Please refer to the Terms of Service from our provider for details.
